Please telephone our office to inquire about fees
and terms for membership enrollment. Upon request,
an order form will be supplied by fax or
email.
PLEASE
NOTE:
Our
members are licensed optometrists who provide
optometric vision therapy (as part of a primary
care practice or exclusively). A pre-screening
interview of the prospective member is conducted
by telephone or in person.
New members who submit a signed order form will
be listed in Find
a Doctor within ten business days and assigned
a November 1 renewal date. If applicable, the
bonus web site will be published within ten business
days following written receipt of the necessary member information as per our one-page instruction sheet.
